Senior Activity List Instructions
Download the Microsoft Word version if you have access to a computer with Word on it and you can email the sheet back to the guidance office.
Download the PDF format if you do not have access to Microsoft Word and email. This form can be printed, filled in, and returned to the Guidance Office.
Instructions:
- List all activities that you have participated in during your high school years. Include any school activities, clubs, sports, etc. Include any office that you have held or will hold your senior year. Include outside activites such as church events, work, volunteer services, etc.
- In the Honors & Awards section, list any honors or awards that you have received both in school and outside of school.
When you have completed the sheet, please return it to Mrs. Powers in the guidance office.
